Consumer Complaint Form
This complaint form is to be used for consumer transactions. A consumer transaction is one in which a person purchases a service or product for their own home or personal use from a person engaged in the business of selling consumer goods or services at retail. The District Attorney's Office does not mediate disputes involving tenant-landlord issues, between two or more individuals, claims against government agencies, or business to business matters.
